When preparing for evaluations at least 10 business days prior to the Provider Certification Review (PCR), the PCR team notifies the provider of the on-site review dates. The on-site review begins with an initial meeting with the provider liaison to discuss the purpose of the review, schedule of review activities, materials needed to complete the review, and persons who should be present at the certification review. Other provider staff may also participate in the initial meeting as determined by the liaison or by request of the Quality Reviewer.
After the discussion, each team member completes a review of the specific individuals who have been included in the sample. The PCR consists of the following activities: observation, site visits, interviewing, documentation review, and staff training/criminal check review.
For additional details on the PCR, please see the DDA Provider Certification Review Policy.
