Emergency Psychologist Affidavit

The Psychologist Affidavit is used to assist individuals with selecting or obtaining an appropriate substitute decision-maker with DDA services who have been assessed to lack the capacity to make or effectively communicate medical decisions for themselves due to psychological issues.

Person-Centered Thinking

Person-centered thinking is a philosophy behind service provision that supports positive control and self-direction of people’s own lives. DDS is working to implement this through training sessions and other agency wide initiatives.
