The purpose ofthis policy is to introduce the use ofa comprehensive and uniform assessment tool designed to provide an assessment of a person's support needs for the person-centered planning process, to identify potential risks to be addressed by the person and hislher planning team, and to provide uniform information upon which the District will make Level of Care determination decisions for eligibility for participation in the ICFIIDD and IDIDD Home and Community-Based Services Waiver programs.

Person-Centered Thinking

Person-centered thinking is a philosophy behind service provision that supports positive control and self-direction of people’s own lives. DDS is working to implement this through training sessions and other agency wide initiatives.
