The purpose of this tool is to report the level of need (LON) which the Department on Disability Services (DDS) uses to review of a determination that a person is ineligible to receive DDA services. The use of a comprehensive and uniform assessment tool is designed to provide an assessment of a person's support needs for the person-centered planning process, to identify potential risks to be addressed by the person and his/her planning team, and to provide uniform information upon which the District will make Level of Care determination decisions for eligibility for participation in the ICFIIDD and IDIDD Home and Community-Based Services Waiver programs.
