HCBS Waiver Service Descriptions and Rules

The Medicaid Home and Community-Based Waiver program requires an application submission process. To become a provider, an application must first submitted for review and approval.

The waiver program provides an alternative to institutional residential services by offering a wide range of daily living, respite, vocational, employment, retirement, social, clinical, therapy and adaptive services and supports in the home and community in a variety of settings.

Types of Supports and Services Provided include Assistive Supports, Day Supports, Professional Services and Residential Supports.

The completed application with original signatures and all required documentation must be mailed to the following address:
