This purpose of the Individual Support Plan (ISP) Meeting Sign In & ISP Request Form is to elect to participate in the HCBS Waiver Program and request personal participation in the development of the ISP plan from DDS.

Person-Centered Thinking

Person-centered thinking is a philosophy behind service provision that supports positive control and self-direction of people’s own lives. DDS is working to implement this through training sessions and other agency wide initiatives.
