Guardianship Routing and Approval Form

The Guardianship Routing and Approval Form is used to begin the request from the Department on Disability Services (DDS), Developmental Disabilities Administration (DDA), when individuals require a substitute decision-maker for individuals receiving supports and services from DDA who have been assessed to lack the capacity to make or effectively communicate medical decisions for themselves.

Person-Centered Thinking

Person-centered thinking is a philosophy behind service provision that supports positive control and self-direction of people’s own lives. DDS is working to implement this through training sessions and other agency wide initiatives.
