Sub Decision Making for Non-Emergency Care Substitute Consent for Health Care Decisions Form

The Substitute Consent for Health Care Decisions Form is used by the individual accepting substitute consent on behalf of individuals with DDA services who have been assessed to lack the capacity to make or effectively communicate medical decisions.

Person-centered thinking is a philosophy behind service provision that supports positive control and self-direction of people’s own lives. DDS is working to implement this through training sessions and other agency wide initiatives.
