Most Integrated Community Based Setting Policy

This form provides DDA with the means to address a complaint against another individual or provider. Either an individual, or any representative, acting on behalf of any individual, may file a complaint. This form may be completed in writing, or may be verbally requested to be completed by this office.

Person-Centered Thinking

Person-centered thinking is a philosophy behind service provision that supports positive control and self-direction of people’s own lives. DDS is working to implement this through training sessions and other agency wide initiatives.

Employment First Initiative
