People with disabilities and their families, advocates, providers and other stakeholders are invited to attend a community forum hosted by the Department on Disability Services (DDS) and the Department of Health Care Finance (DHCF).  We will discuss the District’s updated Statewide Transition Plan for Home and Community Based (HCBS) Waiver Services.  Please join us for a discussion on the following: 

•  Evaluating Our Systems
•  Evaluating Our Settings, and
•  Reviewing the Updated Statewide Transition Plan for HCBS Waiver Services

The updated Statewide Transition Plan for Home and Community Based (HCBS) Waiver Services describes our plan to meet the new HCBS settings requirements by March 17, 2019.
