Your Feedback

In January of 2014, the federal Centers for Medicare and Medicaid (CMS) put out new rules that govern our Home and Community Based Services (HCBS) Waiver for People with Intellectual and Developmental Disabilities. The rules are aimed at making sure people live and spend their day in the community and get services and supports, when they need them just like you or I would. The Riot, which is a newsletter written for and by people with disabilities, has a great issue describing the rule and what it means for people with disabilities.  You can read it here: The Riot   

The District of Columbia has until March 2019 to transform our system of waiver supports to meet the new expectations of the CMS rule.  To begin, we are looking carefully at all of our HCBS Settings (buildings in which people get waiver supports) to better understand where we need to improve.  This is where you come in!  We want to hear from people we support and their families about your experiences. 

Linked below are two surveys: one asks you about your experience with waiver residential supports; and another that asks about your experiences with day supports.  If you receive these supports from DDA, we will also ask you these questions one-on-one.  But, we thought it was important to also give people and their families an opportunity to fill these out and share feedback with us anonymously and with support from whomever you would like.

Your Experiences with Day and Vocational Supports 

This survey asks you about your experience with receiving waiver day and employment (vocational) supports and also about what it is important to you.  It is for people who go a program in a building to participate in Day Habilitation or Employment Readiness supports.  We are hoping to get feedback from people we support and their families.  Thank you!

Your Experiences with Residential Supports

This survey asks you about your experience with receiving waiver residential supports and also about what it is important to you.  It is for people who live in a house or apartment and receive Supported Living, Host Homes, or Residential Habilitation supports.  We are hoping to get feedback from people we support and their families.  Thank you!
