Psychologist Affidavit

The Psychologist Affidavit is used by a psychology professional to verify the request to the Department on Disability Services (DDS), Developmental Disabilities Administration (DDA), when individuals require a substitute decision-maker for individuals receiving supports and services from DDA who have been assessed to lack the capacity to make or effectively communicate medical decisions for themselves.

Person-Centered Thinking

Person-centered thinking is a philosophy behind service provision that supports positive control and self-direction of people’s own lives. DDS is working to implement this through training sessions and other agency wide initiatives.
