On April 22, 2016 the Department on Disability Services identified a data breach involving the personal information of some of our clients. The breach included current and former Rehabilitation Services Administration (RSA) Clients dating back to 2002. This breach was limited to client names and un-formatted social security numbers but contained no protected health or other identifying information. As soon as DDS identified this situation on April 22, we immediately took the following steps to protect DDS client information:
- Removed the offending information.
- Removed all related data from internet search engines.
- Reported this issue to the Office of the Chief Technology Officer (OCTO).
- Launched a thorough review of the agency’s data processes to ensure there are no further incidents of this nature.
In addition, DDS has also reported the breach to the three (3) major credit agencies Equifax,Experian and TransUnion.
