Sub Decision Making for Non-Emergency Care Guardianship Routing and Approval Form

The Guardianship Routing and Approval Form is to apply for Developmental Disabilities Administration (DDA) to assist and individual with selecting or obtaining an appropriate substitute decision-maker for individuals receiving supports and services from DDA who have been assessed to lack the capacity to make or effectively communicate medical decisions for themselves.

Person-Centered Thinking

Person-centered thinking is a philosophy behind service provision that supports positive control and self-direction of people’s own lives. DDS is working to implement this through training sessions and other agency wide initiatives.
