Supported Living Services

Description

Supported Living Service is provided by an agency in a home serving one to three participants that is owned or leased and operated by the agency. Supported Living is a blended rate that covers habilitation, personal care, nursing, and other residential support costs. Supported Living does not included transportation.

Limits and Requirements

Supported Living (Daily): is a daily rate with a limit of 365 days for the person's Waiver certification period. A 24 hour setting for a single participant is only possible when the participant is a danger to others, as determined by psychological assessment and/or court order. The psychological assessment must be updated on an annual basis to determine the continued necessity for this single 24 hour placement.

Supported Living (Periodic): is the same blended service as Supported Living (daily) delivered for less than 18 hours per day. Multiply the number of hours per day  the service will be delivered by the number of days per year service will be delivered. The total should not exceed 6570 hours per year.

Select Supported Living Services* for the rules.
