Supported Employment Services
Description

Intake & Assessment Services are for the development of a detailed formal vocational assessment.

Job Placement Services are those services specific to locating a job and assisting the individual with job preparation and interview tasks.

Job Training & Support (often called job coaching) are those services specific to assisting a person with learning job skills and other capabilities necessary to maintaining employment. This is the service assigned to people who already have jobs.
Limits and Requirements

Intake & Assessment service is limited to 80 hours per year. Frequency Comments should note when the assessment will be completed and delivered. A vocational assessment must be provided with referrals for all other services.

Job Placement service is limited to 100 hours per year. A vocational assessment and documentation that Waiver services are not replacing services provided by RSA are required for this service.

Job Training & Support Professional service is limited to 320 hours per year.

Job Training & Support Paraprofessional service is limited to 320 hours per year.
