Nutritional therapy services shall be designed to maximize independence, prevent further disability, and maintain health. These services should be provided in accordance with the participant’s Plan of Care. All Nutrition Therapy services should be monitored to determine which services are most appropriate to enhance the individual’s well-being and to meet the therapeutic goals.

 

 

If an assessment is needed request four hours of service and note in the Frequency Comments that an assessment is requested and the date the report is to be delivered to DDA. If ongoing therapy services are being requested a schedule of services must be included in an assessment accompanying the request. Ongoing therapy services are billed $55 per hour and the Frequency Comments should provide the schedule of ongoing therapy from the assessment. For persons between the ages of 18 and 21 EPSDT services must be utilized so that Waiver service is not replacing or duplicating EPSDT.
