Occupational Therapy Services
 
Description
 
Occupational therapy services shall be designed to maximize independence, prevent further disability, and maintain health. These services should be provided in accordance with the participant’s Plan of Care. All Occupational Therapy services should be monitored to determine which services are most appropriate to enhance the participant’s well being and to meet the therapeutic goals. This service is delivered by a licensed practitioner and is delivered in the participant’s home or day service setting.
 
Limits and Requirements
 
If an assessment is needed request four hours of service and note in the Frequency Comments that an assessment is requested and the date the report is to be delivered to DDA. An assessment can be requested without a physician's order if the service is identified in the ISP. A physician's order is required for ongoing therapy services. Ongoing therapy services are billed $65 per hour and the Frequency Comments should provide the schedule of ongoing therapy from the assessment and approved by a physician. For persons between the ages of 18 and 21 EPSDT services must be utilized so that Waiver service is not replacing or duplicating EPSDT.
