Respite

Hourly rate is billed at $20.44 per hour and delivered in the person's home.

Daily rate is billed at $310 per day and will include the cost of room and board.

Residential Habilitation

Residential Habilitation is a daily rate with a limit of 365 days for the person's Waiver certification period. Rates are based on the support needs of the individual. The rates are assigned to the person's place of residence. The case manager should verify that the person's address and provider are correct in MCIS so that the correct rate is paid.

Supported Living

Supported Living is a daily rate with a limit of 365 days for the person's Waiver certification period.

Supported Living daily rates are based on the support needs of the individual. The rates are assigned to the person's place of residence. The case manager should verify that the person's address and provider are correct in MCIS so that the correct rate is paid.

In-Home Support

$20.00 per hour.

Live-In Caregiver

Reimbursement rate shall be based on a signed lease agreement and standardized food and utility reimbursement per diem.

Host Home

Host Home rates are based on the acuity or support needs of the individual. The acuity rates are assigned to the person's place of residence. The case manager should verify that the person's address and provider are correct in MCIS so that the correct rate is paid.

Supported Employment		Intake & Assessment Professional: $43 per hour is paid for work done by a Counselor with a Masters Degree in Vocational Counseling or a similar discipline Job Intake & Assessment Paraprofessional $25.30 per hour is paid for work done by a person with a High School or Bachelors degree and experience in supported employment. Job Placement Professional: $43 per hour is paid for work done by a Counselor with a Masters Degree in Vocational Counseling or a similar discipline. Job Placement Paraprofessional: $25.30 per hour paid for work done by a person with a High School or Bachelors degree and experience in supported employment. Job Training & Support Professional: $43 per hour is paid for work done by a Counselor with a Masters Degree in Vocational Counseling or a similar discipline.
